ORDER: Plaintiff's application for leave to proceed in forma pauperis [R. 3] is granted. The Court authorizes and orders the trust fund officer at Plaintiff's place of incarceration to deduct $1.00 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee, and to continue making monthly deductions in accordance with this order. The Court directs the Clerk to: (1) forward copies of this order to the Fiscal Department and to the trust account office at Plaintiff's place of confinement; (2) file Plaintiff's complaint #1 ; (3) issue summonses for service on Defendants; and (4) mail Plaintiff's counsel four blank USM-285 service forms, a magistrate judge consent form, and a copy of this order. The Court appoints the U.S. Marshal to serve Defendants. The U.S. Marshal will not attempt service on Defendants unless and until the required forms are received. The Court grants Plaintiff's motion for attorney representation [R. 4] and requests that Jonathan P. Schaefer / Purcell & Wardrope, Chtd. / 10 South LaSalle Street, Suite 1200 / Chicago, Illinois 60603 / (312) 427-3900 / JSchaefer@pw-law.com represent Plaintiff in accordance with counsel's trial bar commitment under Local Rule 83.37 (N.D. Ill.). Recruited counsel falls within the class of users listed in the Electronic Public Access fee schedule adopted by the Judicial Conference of the United States; therefore, counsel shall be exempt from the payment of fees for access via PACER to the electronic case files maintained in this court for the above-captioned case. The Court instructs recruited counsel to open a separate PACER account to be used only for the above-captioned case, and fees are exempt for this case only He shall not be exempt from the payment of fees incurred in connection with other uses of the PACER system in this court. This exemption is valid immediately and for the duration of counsel's participation in the matter. This exemption may be revoked at the discretion of the Court at any time. Counsel shall contact the PACER Service Center at 1-800-676 6856 or via the link below to create the new PACER account and to make any necessary arrangements for the waiver.
