EMAILED order dated 8/10/2020, #6 to Supervisor of the Inmate Trust Fund Accounts at Cook County Jail and the Court's Fiscal Department. MAILED a copy of the order, an amended civil rights complaint form and instructions, and a blank USM-285 form to Plaintiff. (pj, ) |
Filing 6 ORDER: The Court construes Plaintiff's "prison trust fund account statement" #4 as an application for leave to proceed in forma pauperis; such leave is granted. The initial partial filing fee is waived. The Court orders the trust fund officer at Plaintiff's place of incarceration to make monthly deductions in accordance with this order. The Clerk of Court is directed to send a copy of this order (electronically, if possible) to the trust fund officer at Plaintiff's current place of incarceration and to the Court's Fiscal Department. Summonses, however, shall not issue at this time, as the Court dismisses Plaintiff's complaint #1 without prejudice. Plaintiff is granted until September 8, 2020, to submit an amended complaint and a completed USM-285 (Marshals service) form for each Defendant named in the amended complaint. If Plaintiff does not comply with this order, the Court will dismiss Plaintiff's lawsuit with prejudice for failure to state a claim. Plaintiff must promptly submit a change-of-address notice if he is released or transferred to another facility. If Plaintiff does not send address updates to the Clerk, this case may be subject to dismissal.
