ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied as incomplete. Plaintiff is directed to either (1) submit a renewed and complete application on the Court's form that is certified by a trust fund officer and includes a complete copy of Plaintiff's trust fund statement showing all transactions in his account for the six-month period immediately preceding submission of the renewed application; or (2) pay the full statutory filing fee of $400.00. In any renewed application, Plaintiff must also provide the information described below regarding settlement proceeds in a prior case, Springs v. Schwartz, No. 1:15-cv-5437 (N.D.Ill.) (Ellis, J.). If necessary, Plaintiff may file his IFP Statement under seal by writing "File Under Seal with Permission of the Court" on the top. If he does not comply with these directives by September 30, 2020, the Court will summarily dismiss this case.
