ORDER: Plaintiff has neither prepaid the filing fee nor sought leave to proceed in forma pauperis inthis action. In addition, his handwritten complaint #1 is not on the Court's mandatory form (and does not otherwise provide information required by that form, such as Plaintiff's litigation history). If Plaintiff wants to pursue this litigation, no later than October 13, 2020, he must: (1) pay the full statutory filing fee of $400 or submit an application for leave to proceed in forma pauperis; and (2) submit an amended complaint on the Court's standard form. If Plaintiff does not comply with this order in its entirety by the aforementioned date, this case will be summarily dismissed. Plaintiff must promptly submit a change-of-address notification if he is released or transferred to another facility. If he does not keep the Clerk apprised of his correct address, this case will be subject to dismissal with no further warning. The Clerk is directed to send Plaintiff a blank application for leave to proceed in forma pauperis, an amended complaint form, instructions, and a copy of this order.
