Filing 6 ORDER: For the reasons given below, Plaintiff's application for leave to proceed in forma pauperis, ECF No. #3 , and motion for attorney representation, ECF No. #4 , are denied without prejudice. To proceed with this case, Plaintiff must submit: (1) a proper amended in forma pauperis application that accurately discloses his finances beginning a full year prior to initiating this action, on September 21, 2002, and ending on the date of compliance; and (2) certified trust fund ledgers from any facility where he has been in custody from August 5, 2020 to the date of compliance. Alternatively, Plaintiff may prepay the full $400 filing fee. In either event, Plaintiff also must submit a separate sworn memorandum addressing the apparently false representations in his in forma pauperis application. If Plaintiff does not comply with this order in its entirety by November 16, 2020, the Court will summarily dismiss this case. Given that the U.S. Postal Service returned the case opening letter as undeliverable, the Clerk is directed to update Plaintiff's address to reflect that he is incarcerated at Stateville Correctional Center under prisoner number B-69028. Plaintiff must promptly submit a change-of address notification if he is released or transferred to another facility, as the Clerk is under no obligation to locate him and will not continue to do so. If Plaintiff does not notify the Clerk each time his address changes while this case is open, this case will be subject to dismissal without further warning.
