ORDER: Plaintiff's Application for Leave to Proceed In Forma Pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $21.25 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to make monthly deductions in accordance with this order. The Court directs the Clerk of Court to electronically send a copy of this order to the trust fund officer at Plaintiff's present place of incarceration and to the Court's fiscal department. Summonses, however, shall not issue. Plaintiff's Complaint #1 is dismissed without prejudice for failure to state a claim. If Plaintiff wants to proceed with this lawsuit, he must submit an amended complaint that complies with this order. By November 16, 2020, Plaintiff may submit (1) an amended complaint (if he can do so consistent with Federal Rule of Civil Procedure 11) and (2) a completed USM-285 service form for each Defendant named in the amended complaint. If Plaintiff fails to comply, the Court will dismiss this case for failure to state a claim. The Clerk is directed to send Plaintiff an amended civil rights complaint form, a blank USM-285 form and instructions, and a copy of this order. Plaintiff must promptly submit a change-of address notification if he is transferred to another facility. The Court acknowledges Plaintiff's correspondence #4 and directs the Clerk to forward Plaintiff a copy of his complaint in this case. If Plaintiff wants information from another case (Case No. 20 C 1130) he must file a request in that case. Failure to do so may lead to dismissal of this action for want of prosecution.
