ORDER: On September 29, 2020, Plaintiff submitted a document that details his complaints about his recent arrest and prosecution in the Circuit Court for DuPage County. See Dckt. No. 1. It is unclear from Plaintiff's submission whether he intended to initiate a new lawsuit or if he meant to file the submission in his other open case, Ali v. Unidentified, No. 1:20-cv-5030 (N.D. Ill.) (Norgle, J.). By November 13, 2020, Plaintiff must inform the Court in writing whether he wishes to continue with the present lawsuit. If Plaintiff intended to begin a new lawsuit, by that same date, he must either: (1) submit a completed application for leave to proceed in forma pauperis that includes a certified copy of the trust fund account statement (or institutional equivalent) for the six-month period immediately preceding the filing of the complaint and a completed certification from the appropriate official of each prison where Plaintiff is or was confined during the last six months; or (2) pay the full statutory filing fee of $400. If he does not comply, the Court will dismiss this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis and a copy of this Order.
