ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. To proceed with this lawsuit, Plaintiff must submit a renewed application that (1) is certified by an appropriate official, (2) accurately discloses Plaintiff's income starting twelve months prior to initiating this action, (3) is accompanied by a copy of Plaintiff's trust fund statement showing all transactions in his account for the six-month period immediately preceding initiation of this action, and (4) is signed by Plaintiff. Alternatively, Plaintiff may proceed by paying the full statutory filing fee of $400.00. Plaintiff's motion for attorney representation #4 is denied without prejudice because nothing suggests that Plaintiff is unable to respond to this order, which calls only for basic financial information. If this case proceeds, Plaintiff may renew his motion for attorney representation at a later date. Failure to comply with this order by January 14, 2021, will result in summary dismissal of this case. Plaintiff also must promptly submit a change-of-address notification if he is transferred to another facility or released. Failure to keep the Court informed of Plaintiff's address may lead to dismissal of this action for failure to comply with a Court order and for want of prosecution.
