Filing 7 ORDER: Signed by the Honorable Franklin U. Valderrama on 2/11/2021.Tommy Lee Williams, Jr., who is no longer incarcerated, but was a detainee at the Cook County Jail when he initiated this action, brings this pro se civil rights action under 42 U.S.C. 1983 concerning his October 26, 2020 arrest by Chicago Police officers. Before the Court are Plaintiff's application for leave to proceed in forma pauperis, complaint for initial review, and motion for attorney representation. Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to deduct $6.00 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Court directs the Clerk of Court to (1) send (electronically if possible) a copy of this order to the trust fund officer at Plaintiff's current place of incarceration and to the Court's Fiscal Department; (2) file Plaintiff's complaint #1 ; (3) issue summons for service of the complaint on Defendant Mohammed by the U.S. Marshal; and (4) send Plaintiff one blank USM-285 (Marshals service) form, a magistrate judge consent form, filing instructions, and a copy of this order; and (5) change Plaintiff's address to: 7242 Oglesby Avenue, Chicago, IL 60649. Plaintiff must complete and return a USM-285 form for service on Defendant. Failure to return the USM-285 form may result in dismissal of this case in its entirety. Plaintiff also must promptly submit a change-of-address notification if his residence changes. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute. The Court appoints the U.S. Marshal to serve Defendants. Plaintiff's motion for attorney representation #4 is denied without prejudice to renewal later in this case. Mailed notice (ph, ) |
