April 29, 2021 Opinion or Order Filing 6 ORDER: Plaintiff is currently on electronic monitoring from Cook County Jail. His Application to Proceed In Forma Pauperis #3 is denied without prejudice because it is incomplete. Plaintiff must file a renewed in forma pauperis application that accurately discloses his finances starting a full year before he initiated this lawsuit (in other words, from approximately April 2020 to the date he responds). He must also attach a certified copy of his Cook County Jail trust fund account ledgers for any period of time that he was detained within that period. The Court will use this information to calculate what amount of money (if any) Plaintiff must pay toward the filing fee. Alternatively, Plaintiff may pay the full $402 filing fee in advance. If Plaintiff does not comply by May 20, 2021, this action will be summarily dismissed. The Clerk is directed to send a blank application to proceed in forma pauperis for non-incarcerated persons to Plaintiff, along with a copy of this order. The Clerk is directed to update Plaintiff's address on the docket to: 7805 S. Calumet Ave., Chicago, IL 60619. Plaintiff is advised that he has a continuing obligation to keep the Court apprised of his current address, and failure to do so may result in dismissal of this case.
