Filing 5 ORDER: The Court denies Plaintiff's application for leave to proceed in forma pauperis #3 without prejudice. If Plaintiff wants the Court to screen his complaint, he must file a properly supported amended in forma pauperis application that accurately discloses his income starting twelve months prior to initiating this action (in other words, approximately May 1, 2020) and ending on the date of compliance. He must also include a trust fund certification completed by an authorized correctional official with supporting trust fund ledgers beginning on November 1, 2020, and ending on the date of compliance. Alternatively, Plaintiff may prepay the full $402 filing fee. If Plaintiff does not comply by June 10, 2021, the Court will summarily dismiss this case. Plaintiff's motion for attorney representation #4 is denied without prejudice as he did not answer the question on the Court's form requiring him to provide specifics about what he has done to attempt to obtain counsel himself and whether an indigent litigant has made a reasonable attempt to obtain his own attorney is a "mandatory, threshold inquiry," Eagan v. Dempsey, 987 F.3d 667 (7th Cir. 2021); in any event, nothing suggests that Plaintiff is unable to address his fee status on his own. Plaintiff must promptly update his address if he is released or transferred to another facility. If he fails to do so, this case will be subject to dismissal with no further warning.
