Filing 5 MINUTE entry before the Honorable John F. Kness: Pending before the Court is Plaintiff's application #4 for leave to proceed in forma pauperis ("IFP"). Because the application is incomplete, the Court orders Plaintiff to submit an amended application on or before 07/16/2021. Specifically, the present application fails to provide sufficient information concerning Plaintiff's current income and assets. For example, in Paragraph 3, Plaintiff refers to a joint account with a relative, but it is unclear whether any of the money in that account is for the benefit of Plaintiff. In addition, Plaintiff does not provide any specifics concerning the amount of benefits (if any) he receives in the form of the public assistance he states he receives. In Paragraph 4, Plaintiff states that he has money in a bank account, but he does not describe, as required by the form, how much money is in the account. Plaintiff must address each of these issues in the amended application before the Court can issue a ruling. Plaintiff's counsel must assist Plaintiff in preparing the amended application and must ensure that it is submitted to the Court ex parte and under seal on or before the deadline.
