ORDER: This matter is before the Court on Plaintiffs' applications for leave to proceed in forma pauperis, #5 and #6 . Both applications #5 and #6 , are denied without prejudice because neither Plaintiff has responded to question 2, concerning whether the Plaintiff is married and, if so, the spouse's employment status and income. The Court needs this information to assess whether Plaintiffs are entitled to have the filing fee waived. Alternatively, if Plaintiffs do not wish to disclose this information, they may pay the filing fee. In addition, Plaintiffs' pro se appearance forms, #3 and #4 , are incomplete. Plaintiffs fail to provide their street addresses or telephone numbers. This information is required, even when a plaintiff has opted to receive notice through email. Plaintiffs are given until October 6, 2021 to (1) file revised, complete applications for leave to proceed in forma pauperis or, alternatively, pay the filing fee; and (2) file revised, complete pro se appearance forms, including their addresses and telephone numbers.
