Filing 8 ORDER: Plaintiff 's Application for Leave to Proceed In Forma Pauperis #4 is denied without prejudice. Plaintiff is directed to either (1) submit a renewed application that is certified by a trust fund officer and that includes a copy of Plaintiff's prison trust fund statement showing activity in his account for the six months immediately preceding his filing of his complaint; or (2) pay the full statutory filing fee of $402.00. Failure to comply with this order by October 27, 2021, will result in summary dismissal of this case. The Clerk of Court is directed to: (1) send Plaintiff a blank application to proceed in forma pauperis and a copy of this order; and (2) send a copy of this order to the trust fund officer at the facility having custody of Plaintiff, so that deductions may be taken for the fees owed in Junious v. CCDOC, No. 19 C 3964 (N.D. Ill.), and Junious v. IDOC, No. 19 C 3974 (N.D. Ill.), as outlined in this order. Plaintiff's Motion for Recruitment of Counsel, Motion for Appointment of Counsel, and Motion for Service of Process at Government Expense #5 #6 #7 are denied without prejudice.
