ORDER: Illinois prisoner Montez Artis initiated this action under 42 U.S.C. 1983, but he did not pay the Court's filing fee or apply for leave to proceed in forma pauperis. If Plaintiff wants to proceed to screening under 28 U.S.C. 1915A, he has until December 13, 2021, to either (1) submit an application to proceed in forma pauperis on the enclosed form along with a copy of his trust fund account statement showing all transactions to his account for the twelve-month period immediately preceding his initiation of this action (from October 17, 2020 to October 17, 2021); or (2) pre-pay the $402.00 filing fee. Failure to comply with this order will result in summary dismissal of this case.
