It is not clear that the particular restaurant location is the proper place for service, and it might be sensible for the Plaintiff to contact the Pro Se Help Desk, and can submit an amended USM-285 if appropriate. December 28, 2021 Filing 6 MINUTE entry before the Honorable Edmond E. Chang. On review of the motion #4 to proceed in forma pauperis, the motion is granted in light of financial indigency. The Clerk's Office shall file the complaint. The motion #5 for attorney representation, however, is denied for now but without prejudice. The motion notes that the Plaintiff has some college education, and for the moment, the next step is just service of process by filling out the USM-285 service form. To that end, the Clerk's Office shall issue summons and mail a blank USM-285 service form to the Plaintiff, who must fill it out and mail it back to the U.S. Marshals Service (located on the 24th Floor of the Courthouse, 219 South Dearborn Street, Chicago, Illinois, 60604) by 01/25/2022. The Plaintiff also may ask the Pro Se Help Desk for assistance in filling out the form (if needed). The Marshals Service is appointed to serve the summons and complaint (after receipt of the USM-285), and may first seek a waiver of service under Civil Rule 4(d).
