Ellis initiated this action under 42 U.S.C. 1983, but he did not pay the Court's filing fee or apply for leave to proceed in forma pauperis. If Plaintiff wants to proceed further, he has until January 20, 2022, to either (1) prepay the $402.00 filing fee; or (2) submit an application to proceed in forma pauperis on the enclosed form along with a copy of his trust fund account statement showing all transactions to his account for the twelve-month period immediately preceding his initiation of this action (from November 27, 2020, to November 27, 2021). Failure to comply with this order will result in dismissal of this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed in forma pauperis and a copy of this order.
