ORDER: Plaintiff's application to proceed in forma pauperis #3 is denied without prejudice because it is incomplete. If Plaintiff wishes to proceed with this lawsuit, he must either: (1) submit a renewed application for leave to proceed in forma pauperis as detailed in this order; or (2) pre-pay the statutory $402 filing fee. Plaintiff has until 2/10/2022 to comply. Failure to comply will result in the dismissal of this lawsuit. The Clerk is directed to send Plaintiff a blank application to proceed in forma pauperis and a copy of this order at: (1) Plaintiff's current address identified on the docket (at Stateville); and (2) Plaintiff's last known private address on file with the Clerk's Office (2204 N.
