ORDER Signed by the Honorable Andrea R. Wood on 2/17/2022: Pursuant to the Court's order dated 1/19/2022 #7 , Plaintiff has submitted a completed in forma pauperis application using this District's form. Having reviewed Plaintiff's submission, the Court finds that he is unable to pay the fees and costs associated with this action, and so his application to proceed in forma pauperis #8 is granted. The filing fee is waived. The U.S. Marshal is appointed to serve Defendants. The Clerk is directed to issue summonses for service of the Complaint #2 on Defendants by the U.S. Marshal and to send Plaintiff a blank USM-285 service form for each Defendant, a magistrate judge consent form, filing instructions, and a copy of this Order. The Court advises Plaintiff that a completed USM-285 form is required for service on each Defendant. Alternatively, Plaintiff may elect to serve Defendants or request waivers of service on his own. Telephonic status hearing set for 3/31/2022 at 10:00 AM. To ensure public access to court proceedings, members of the public and media may call in to listen to telephonic hearings. The call-in number is (888) 557-8511 and the access code is 3547847. Counsel of record and Plaintiff (if an email address has been provided to the Court) will receive an email 30 minutes prior to the start of the telephonic hearing with instructions to join the call. Persons granted remote access to proceedings are reminded of the general prohibition against photographing, recording, and rebroadcasting of court proceedings. Violation of these prohibitions may result in sanctions, including removal of court-issued media credentials, restricted entry to future hearings, denial of entry to future hearings, or any other sanctions deemed necessary by the Court. ORDER Signed by the Honorable Andrea R. Wood on 1/19/2022: This case has been transferred from the United States District Court for the Southern District of New York to the United States District Court for the Northern District of Illinois. At the time of the transfer, Plaintiff's application for leave to proceed in forma pauperis #1 , filed on the Southern District of New York's form, was pending. The application is hereby denied without prejudice because it is incomplete. Specifically, Plaintiff has failed to answer fully Questions (2) and (3). If Plaintiff wishes to proceed with his case in this Court, he will need to submit a new in forma pauperis application using the Northern District of Illinois's form, making sure to provide complete answers to all questions (including any subparts). Further, Plaintiff may be eligible to have counsel recruited to represent him pro bono (i.e., at no cost to him) in connection with his case. If Plaintiff wishes to make a request for recruited counsel, he should fill out a motion for attorney representation form and submit it with his completed in forma pauperis application.
