If Plaintiff wants to proceed further, he has until June 2, 2022, to either (1) prepay the $402.00 filing fee; or (2) submit an application to proceed in forma pauperis on the enclosed form along with a copy of his trust fund account statement showing all transactions to his account for the six-month period immediately preceding his initiation of this lawsuit (from November 2, 2021 to May 2, 2022).
