ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. If he wants to proceed with this case, Plaintiff must: (1) submit a properly completed and certified application as set forth below or pre-pay the $402.00 filing fee; and (2) show cause (explain in writing) why this Court should not impose sanctions, up to and including dismissal of this case, for forgery of the trust fund certificate. Failure to fully comply with this order by June 10, 2022, will result in dismissal of this case. Plaintiff's motion for attorney representation #4 is denied without prejudice.
