August 22, 2022 Opinion or Order Filing 5 ORDER: The Court grants Plaintiff's motion for leave to proceed in forma pauperis #3 , dismisses defendants "unknown medical doctor" and "unknown eye doctor" from the complaint #1 for misjoinder with leave to amend. The Court also dismisses defendant Cook County Jail #1 and denies without prejudice his motion for attorney representation #4 . The Court authorizes and orders the trust fund officer at Plaintiff's place of incarceration to deduct $12.67 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee, and to continue making monthly deductions in accordance with this order. Plaintiff's trust fund officer must collect 80 percent of all income into his trust account (in reducing increments of 20 percent as statutory fees are successively paid off) until accrued filing fees are paid. Plaintiff is granted until September 26, 2022, to submit: (1) if Plaintiff so chooses, an amended complaint on the Court's required form, as detailed in this order and (2) a completed USM-285 (Marshals service) form for each Defendant named in the amended complaint. The Clerk of Court is directed to send: (1) a copy of this order to Plaintiff, along with an amended civil rights complaint form and a blank USM-285 form and (2) a copy of this order (electronically if possible) to the trust fund officer of the facility having custody of Plaintiff and to this Court's Fiscal Department. Plaintiff must promptly update his address if it changes while this case remains open. If he fails to do so, this case will be subject to dismissal with no further warning.
