Filing 9 MINUTE entry before the Honorable Matthew F. Kennelly: Plaintiff's amended application to proceed in forma pauperis #8 is granted. Plaintiff has filed a lawsuit against Equifax for what appear to be alleged violations of the Fair Credit Reporting Act (FCRA). Plaintiff used the wrong pro se complaint form for this; he used a form for constitutional rights violations. But the form does not matter at this point. Plaintiff alleges that Equifax erroneously reported accounts supposedly related to him; he asked Equifax to correct the errors but it did nothing; and he was denied credit as a result. This is sufficient for present purposes. The Clerk is directed to issue summons. The US Marshal is appointed to serve the defendant with summons and the complaint. The Clerk is directed to send plaintiff a USM-285 form. Plaintiff is advised that this is the form that the Marshal will use to serve Equifax with summons. Plaintiff is directed to fill out the form completely and return it to the Clerk by no later than 8/24/2022. The case is set for a telephonic status hearing on 10/25/2022 at 8:45 AM. The following call-in number will be used for the hearing: 888-684-8852, conference code 746-1053. Plaintiff is directed to call in for the status hearing at that date and time. Persons granted remote access to proceedings are reminded of the general prohibition against photographing, recording, and rebroadcasting of court proceedings. Violation of these prohibitions may result in sanctions, including removal of court issued media credentials, restricted entry to future hearings, denial of entry to future hearings, or any other sanctions deemed necessary by the Court.
