ORDER: Plaintiff's renewed application for leave to proceed in forma pauperis #8 is granted. The Court orders the trust fund officer at Plaintiff's place of incarceration to immediately deduct $7.47 from Plaintiff's account for payment to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Court directs the Clerk of Court to (1) send a copy of this order to the trust fund officer of the facility having custody of Plaintiff and to the Court's Fiscal Department; (2) file Plaintiff's amended complaint #7 ; (3) issue summonses for service of the complaint on Defendants Dr. Aguinaldo and Nurse Tina by the U.S. Marshal; (4) send Plaintiff two blank USM-285 (Marshals service) forms; and (5) terminate the remaining Defendants. Plaintiff must complete and return a USM-285 form for service on each Defendant. Plaintiff's motions for attorney representation #4 #9 are granted. The Court recruits David M. Neff, Perkins Coie LLP, 110 North Wacker Drive, Suite 3400, Chicago IL 60606, (312) 324-8400, dneff@perkinscoie.com, to represent Plaintiff in this case in accordance with counsel's trial bar obligations under N.D. Ill. Local Rules 83.11(h) and 83.37. 1 Counsel is directed to file an appearance by October 20, 2022, and a status report regarding her investigation of this matter by November 18, 2022. If counsel believes that proceeding with this representation would violate her Rule 11 obligations, she should inform the Court. The Court notifies Plaintiff's counsel that pro bono litigation resources are available on the District's website at http://www.ilnd.uscourts.gov, by clicking on the link to Resources for Pro Bono Attorneys in the Trial Bar. Another resource is an attorney with the Uptown People's Law Center, Mr. Alan Mills, who may be reached at alan@uplcchicago.org or 773-250-0357. In addition, counsel should review Local Rule 54.1 for the rules addressing the taxation of costs and the limits established by the Judicial Conference of the United States and/or published rates on the Court's website. Plaintiff must either (1) pay the full statutory filing fee of $402.00 or (2) submit a renewed application to proceed in forma pauperis that complies with the instructions in this order.
