The claims against Maywood Police Dept., which is not a suable entity, are dismissed. The U.S. Marshals Service is appointed to serve remaining Defendants. The Clerk shall send three USM-285 forms to Plaintiff. Plaintiff has until 10/13/2022 to complete the USM-285 forms (one for each remaining Defendant) and return them to the Clerk of Court. Failure to comply will result in the dismissal of this lawsuit. Moreover, Plaintiff represents that she has received no income of any kind over the past 12 months and has no assets, and yet further represents that she provides "100% support" to her daughter, son, and grandchildren. Plaintiff has until 9/14/2022 to file a complete and factually accurate in forma pauperis application. If Plaintiff again represents that she has received no income of any kind over the past 12 months and has no assets, she should explain how she is able to support herself, her children, and her grandchildren. Motion for attorney representation #5 is denied without prejudice. Plaintiff's motion indicates that she has not made any effort, let alone a sufficient effort, to find representation on her own. Also, Plaintiff is a college graduate and thus should have the ability to represent herself at this early stage of the case.
