ORDER: Plaintiff's application to proceed in form pauperis in Case No. 22 C 5726 #3 is granted. The trust fund officer at Plaintiff's place of incarceration is authorized to deduct $10.43 from Plaintiff's account as the initial partial payment of the filing fee and to continue making deductions in accordance with this order as set forth below. Plaintiff's application to proceed in form pauperis #3 and motion for attorney representation #4 in Case No. 22 C 5728 are denied and that case is closed as duplicative of Case No. 22 C 5726. No filing fee is assessed in the duplicate case. However, summonses shall not issue at this time in Case No. 22 C 5726. Plaintiff is given until December 8, 2022, to submit an amended complaint in Case No. 22 C 5726 that includes his claims against both King Gaston and Odell Jamison. The Clerk is directed to send a copy of this order to Plaintiff, the Court's Fiscal Department, and to Joliet Treatment Center. The Clerk shall also send Plaintiff an amended complaint form and two blank USM-285 forms. If Plaintiff fails to submit an amended complaint by the deadline, the Court will proceed with the initial review of this case (22 C 5726) as to only King Gaston. Signed by the Honorable Manish S.
