Filing 4 APPLICATION by Plaintiff Khari Walker for leave to proceed in forma pauperis. (Exhibits). (Envelope not postmarked). (jn, ) |
MAILED order dated 11/15/2022 #3 and a blank application to proceed In Forma Pauperis to Khari Walker. (jn, ) |
Filing 3 ORDER: Pro se Plaintiff Khari Walker, an inmate at the Cook County Jail, initiated this civil rights lawsuit without paying the Court's filing fee or applying for leave to proceed in forma pauperis (IFP). The Prison Litigation Reform Act (PLRA) requires that all prisoners seeking to bring a federal civil action or appeal pay the full filing fee. See 28 U.S.C. 1915(b). A prisoner may pay the fee in full in advance or, if qualified to proceed IFP, make installment payments deducted directly from his custodial trust fund account. Thus, if Plaintiff wishes to proceed further with this lawsuit, he has until December 15, 2022, to either (1) file a properly completed and supported application to proceed IFP on the enclosed form, or (2) prepay the $402 filing fee. Failure to comply with this order by the aforementioned date will result in summary dismissal of this case. The Clerk of Court is directed to send Plaintiff a blank application to proceed IFP and a copy of this order. Signed by the Honorable Franklin U. Valderrama on 11/15/2022. Mailed notice (jn, ) |
MAILED copy of the Clerk's Notice entry along with the Joint Consent form to Plaintiff Khari Walker. (jn, ) Modified on 11/10/2022 (jn, ). |
