Plaintiff is directed to either (1) submit a renewed application in accordance with this order; or (2) pay the full statutory filing fee of $402.00. The Clerk is directed to send Plaintiff a blank application to proceed in forma pauperis. The Clerk of Court is also directed to send a copy of this order to the trust fund account officer and to the facility director at Elgin Mental Health Center. These officials are directed to provide Plaintiff with certified trust fund account information for May 22, 2022 through November 22, 2022. Plaintiff's failure to comply with this order by January 5, 2023, will result in dismissal of this case.
