ORDER: For the reasons discussed below, this case is dismissed without prejudice. All pending motions (e.g., #4 , #5 ) are terminated as moot. The court refers this matter to this court's Executive Committee with a recommendation that all pending matters for the Samuel Sledge who resides at 907 North Lawler, Chicago, Illinois 60651, be stayed until the Executive Committee can (1) determine whether he initiated this lawsuit on behalf of the incarcerated Mr. Sledge; and, if so (2) impose an appropriate sanction for the unauthorized practice of law in this court. ORDER: The Court orders Plaintiff to clarify (under oath and in writing) his relationship to the Samuel Sledge who resides at 907 North Lawler, Chicago, Illinois 60651. Plaintiff also must advise whether he authorized initiation of this lawsuit, who prepared the documents received by the Court, and whether he wants to proceed further. Plaintiff's failure to respond to this order will result in referral of this matter to this Court's Executive Committee with a recommendation that all pending matters for both Plaintiff and the Samuel Sledge who resides at 907 North Lawler, Chicago, Illinois 60651, be stayed until the matter is resolved. The Court also will assess and collect the $402.00 filing fee for this case from the incarcerated Mr. Sledge consistent with the provisions of 28 U.S.C. 1915(b). The Court directs the Clerk of Court to change the "Nature of Suit" to 550 before sending a copy of this order to Plaintiff at the custodial address reflected on the docket.
