ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders Plaintiff (and his trust fund officer) to pay $13.23 as an initial partial filing fee. The Court further directs Plaintiff's trust fund officer to begin making monthly deductions from Plaintiff's account toward payment of the filing fee in accordance with this order. The Court directs the Clerk of Court to: (1) send a copy of this order (electronically, if possible) to Plaintiff's trust fund officer and to the Court's Fiscal Department; (2) file Plaintiff's complaint #1 ; (3) issue summons for service of the complaint on Defendant Physician Assistant Martinez by the U.S. Marshal; (4) dismiss all other Defendants; and (5) send Plaintiff (at his last known address) a blank USM-285 (Marshals service) form, filing instructions, and a copy of this order. The Court appoints the U.S. Marshal to serve Defendant Martinez. Plaintiff must complete and return a USM-285 form for service on Defendant. Failure to return the USM-285 may result in dismissal of Martinez as a Defendant, as well as dismissal of this case in its entirety. Plaintiff must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute. Plaintiff's motion for attorney representation #4 is granted. The Court requests that Andrew George Klevorn of Andrew Klevorn, Esq., 6250 N. Glenwood Avenue, Chicago, IL 60660, represent Plaintiff in this case in accordance with counsel's trial bar obligations under the District Court's Local Rules 83.11 and 83.37 (N.D. Ill.). Counsel is directed to file an appearance in this case by March 1, 2023.
