For example, the application does not include financial obligations such as rent or sources of funds used for living expenses. Plaintiff also did not answer questions about his date of last employment, last monthly take-home pay, debts and financial obligations, and additional information. Plaintiff is directed to either pay the filing fee or submit a completed in forma pauperis application by 4/20/2023. Plaintiff is advised that a new application must include an answer to every question. In other words, there can be no blanks. Plaintiff should check a box for each question.
