If Plaintiff wants to pursue this lawsuit, he must either (1) pre-pay the $402.00 filing fee, or (2) submit a renewed application that contains all the information specified in this order, including a copy of his trust fund statements for the entire six-month period preceding the filing of this lawsuit. Failure to comply with this order by May 18, 2023, will result in summary dismissal of this case. Plaintiff's motion for attorney representation #4 is denied without prejudice. The Clerk is directed to: (1) correct Warden Charles Truitt's name on the docket; and (2) send Plaintiff a blank application for leave to proceed in forma pauperis (prisoner version) and a copy of this order.
