Filing 4 ORDER: The Court grants Plaintiff's application for leave to proceed in forma pauperis #3 and orders the trust fund officer at Plaintiff's place of confinement to deduct $21.17 from Plaintiff's account for payment to the Clerk as an initial partial payment of the filing fee, and to continue making monthly deductions in accordance with this order. The Clerk is directed to: (1) send a copy of this order to the Cook County Jail's trust fund officer; (2) file Plaintiff's complaint #1 ; (3) issue summonses for service of the complaint #1 on Sergeant Penderson and Officers Hejaz and Davis; (4) correct the docket to list Officer Yusif and Officer Useif as a single person ("Officer Yusif a/k/a Useif") and then terminate that individual as an active defendant; and (5) send Plaintiff three blank USM-285 service forms and a copy of this order. Plaintiff must complete USM-285 service forms for Sergeant Penderson and Officers Hejaz and Davis and return them to the Clerk in care of the Prisoner Correspondent. He should provide as much identifying information as possible (physical description, full name, badge number, etc.). If Plaintiff fails to comply by June 2, 2023, this case will be summarily dismissed. The Marshals Service is appointed to effectuate service and is authorized to send requests for waiver of service before attempting personal service. Plaintiff must promptly update his address if it changes while this case is open. If he fails to do so, this lawsuit will be subject to summary dismissal with no further warning.
