ORDER: By order of May 24, 2023, Judge Manish S. Shah, who previously presided over this case, denied Plaintiff's application for leave to proceed in forma pauperis without prejudice #6 . Plaintiff was directed to either pre-pay the $402.00 filing fee or submit a renewed application containing all the information specified in that order if he wanted to proceed with this case. Plaintiff was warned that failure to comply by June 28, 2023, would result in dismissal of this case. To date, Plaintiff has not complied with the order. He instead submitted a "Notice of Intention to Disclose Litigation History" and a notice of change-of-address indicating that he has been transferred to Dixon Correctional Center #8 #9 . Plaintiff's submissions make no mention of his fee obligations. On its own motion, the Court extends the deadline for compliance with the fee order to July 28, 2023. Plaintiff's failure to either pre-pay the $402.00 filing fee or submit a properly completed in forma pauperis application by that date will result in summary dismissal of this case. The Clerk is directed to (1) correct Plaintiff's inmate number on the docket to the one provided in the caption of this order; and (2) send Plaintiff a copy of this order, a copy of the May 24, 2023, order #6 and a blank in forma pauperis application. If Plaintiff wants to pursue this lawsuit, he must either (1) prepay the $402.00 filing fee, or (2) submit a renewed application that contains all the information specified in this order, including a copy of his trust fund statements for the entire six-month period preceding the filing of this lawsuit. Failure to comply with this order by June 28, 2023, will result in summary dismissal of this case. Plaintiff's motion for attorney representation #4 is denied without prejudice.
