Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. If Plaintiff wants to proceed with this lawsuit without prepaying the statutory filing fee, he must submit a renewed, properly completed application that is certified by an appropriate official and accompanied by an unredacted copy of his jail trust fund statement. The trust fund ledger must show all transactions in Plaintiff's account for the six-month period immediately preceding his initiation of this action (that is, from November 25, 2022, through May 25, 2023). The fill-in-the- blank section must provide an accurate accounting of his income from all sources, with the payors not crossed out. In the alternative, Plaintiff may choose to pay the full statutory filing fee of $402.00. Failure to comply with this order by August 9, 2023, will result in summary dismissal of this case. The Court directs the Clerk to mail Plaintiff a blank IFP form and a copy of this order. Plaintiff must be truthful, accurate, and thorough in all of his submissions.
