Filing 7 ORDER: Plaintiff's application for leave to proceed in forma pauperis #4 is granted. The Court orders Plaintiff (and his trust fund officer) to pay $26.09 as an initial partial filing fee. The Court further directs Plaintiff's trust fund officer to begin making monthly deductions from Plaintiff's account toward payment of the filing fee in accordance with this order (60% of Plaintiff's income, when it meets statutory minimums, must be devoted toward his filing fees). The Court directs the Clerk of Court to: (1) send a copy of this order (electronically, if possible) to Plaintiff's trust fund officer and to the Court's Fiscal Department; (2) file the complaint; (3) issue summonses for service of the complaint on Defendants Mack, Borrero, Szul, James, Hernandez, and the County of Cook; (4) dismiss all other Defendants; (5) send the USM-285 forms received by Plaintiff 6 to the U.S. Marshal; and (6) send Plaintiff a copy of this order. The Court appoints the U.S. Marshal to serve Defendants using the USM-285 forms returned by Plaintiff. The U.S. Marshal is authorized to send a request for waiver of service to Defendant in the manner prescribed by Federal Rule of Civil Procedure 4(d) before attempting personal service. Plaintiff must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute.
