Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is granted. The Court orders Plaintiff (and his trust fund officer) to pay $15.00 as an initial partial filing fee. The trust fund officer at Plaintiff's place of incarceration is authorized and ordered to make further deductions in accordance with this order. Plaintiff may proceed on his complaint. The Court directs the Clerk of Court to: (1) file the complaint #1 ; (2) change the case caption to reflect that Plaintiff's surname is spelled "Odrowski;" (3) terminate David Adams, Vincent Perillo, Deputy Chief Burse, and the City of Joliet as Defendants pursuant to 28 U.S.C. 1915A; (4) send a copy of this order (electronically, if possible) to Plaintiff's trust fund officer and to the Court's Fiscal Department; (5) issue summonses for service of the complaint on Defendants Mort, McDonald, and Will County by the U.S. Marshal; and (6) send Plaintiff three blank USM-285 (Marshals service) forms, filing instructions, and a copy of this order. The Court appoints the U.S. Marshal to serve Defendants. Plaintiff must complete and return USM-285 forms for service on Defendants. Failure to return the USM-285s by November 15, 2023, may result in dismissal of unserved Defendants, as well as dismissal of this case in its entirety. Plaintiff must promptly submit a change-of-address notification if he is transferred to another facility or released. If Plaintiff fails to keep the Court informed of his address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute.
