October 17, 2023 Opinion or Order Filing 9 ORDER: On plaintiff's application to proceed in forma pauperis #6 , which the court previously granted, #7 , plaintiff checked a box indicating that the application was in support of a request for an attorney. However, plaintiff did not file a separate motion for attorney representation. To the extent that plaintiff would like to request an attorney, plaintiff may file such a request by completing and filing the court's form Motion for Attorney Representation. That form and others are available on the court's website by visiting https://www.ilnd.uscourts.gov/Default.aspx, clicking on "Information for People without Lawyers," and clicking on "Federal Civil Case Forms"). No such motion is pending, and this order does not reach the issue of attorney representation or in any way indicate how the court would rule on such a motion if it were filed. The Court advises Plaintiff that a completed USM-285 form for each Defendant is required for service. If Plaintiff has questions about proceeding pro se or searching for an attorney, Plaintiff may contact the Hibbler Help Desk, which can be reached at (312) 702-8825, or www.ilnd.uscourts.gov, and click on the link Information for People Without Lawyers. September 29, 2023 Filing 6 APPLICATION by Plaintiff Kimyuna Jackson for leave to proceed in forma pauperis. (rc, )
September 22, 2023 Opinion or Order Filing 5 ORDER: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice to renewal. Plaintiff's IFP application appears to be either inaccurate or incomplete, and thus does not enable the court to determine whether plaintiff lacks the means to pay the filing fee. Plaintiff must use her best efforts to legibly fill out the application completely and accurately, under penalty of perjury, and to follow the instructions on the form to answer every question, not leave blanks, and if the answer is "0" or "none," say so. Courts rely on litigants to provide complete and honest information about their finances, and judges cannot merely rubber- stamp IFP applications that appear to contain incomplete, inaccurate, or inconsistent information. See Campbell v. Clarke, 481 F.3d 967, 970 (7th Cir. 2007). As the IFP application form states, a false statement may result in dismissal. See 28 U.S.C. 1915(e)(2)(A). If Plaintiff chooses to proceed with this case, she must by 10/10/2023 file a properly supported amended in forma pauperis application that accurately discloses all her income, assets, and financial obligations, starting twelve months prior to initiating this action and ending on the date of compliance. Alternatively, Plaintiff may prepay the $402 filing fee. Signed by the Honorable Martha M. Pacold on 9/22/2023: Mailed notice. (rc, )
