ORDER: Plaintiff Dominque Beck's (Beck) application for leave to proceed in forma pauperis #4 is granted. The Court orders Beck (and her trust fund officer) to pay $22.50 as an initial partial filing fee. The Court further directs Beck's trust fund officer to begin making monthly deductions from Beck's account toward payment of the filing fee in accordance with this order. The Court directs the Clerk of Court to: (1) send a copy of this order (electronically, if possible) to Beck's trust fund officer and to the Court's Fiscal Department; (2) file the complaint; (3) issue summons for service of the complaint on Defendant Dart; and (4) send Beck a copy of this order and a blank USM-285 Form. Beck must return a completed USM-285 Form by November 13, 2023, or the matter may be dismissed for failure to prosecute. The Court appoints the U.S. Marshal to serve Defendant. The U.S. Marshal is authorized to send a request for waiver of service to Defendant in the manner prescribed by Federal Rule of Civil Procedure 4(d) before attempting personal service. Beck must promptly submit a change-of-address notification if she is transferred to another facility or released. If Beck fails to keep the Court informed of her address, this action will be subject to dismissal for failure to comply with a Court order and for failure to prosecute.
