Filing 5 ORDER: Plaintiff has initiated this case about a 12/12/23 incident where two Cook County Jail officers allegedly slammed him to the ground and twisted his arm in a way that injured his shoulder and lower back. #1 . He seeks to proceed in forma pauperis (IFP) for the Court's $405 filing fee in order to pay the fee with monthly deductions from his jail trust fund account. See 28 U.S.C. 1915(b)(1)-(2). His IFP application, however, is incomplete: pages are missing; the certificate section, which must be completed by an authorized officer at his facility, appears to have been filled out by Plaintiff; and no six-month trust account statement is included. His IFP application #3 is denied without prejudice to Plaintiff submitting a completed application or paying the filing fee upfront.1 The Clerk of Court shall send an IFP application to Plaintiff, along with this order. Plaintiff's failure to comply with this order by February 14, 2024, will result in dismissal of this case without prejudice. See N.D. Ill. Local Rule 3.3(f). Plaintiff's motion for attorney representation #4 is denied at this time. Not only has Plaintiff not demonstrated that he attempted to obtain such representation on his own, but also it is too early in the case for the Court to assess whether an attorney is needed.
