Filing 4 ORDER: Plaintiff brings this 42 U.S.C. 1983 action concerning his inability to obtain medications at the Cook County Jail. He has, however, neither prepaid the $405 filing fee, nor submitted an in forma pauperis (IFP) application seeking to pay the fee with monthly deductions from his jail trust fund account. See 28 U.S.C. 1915(b)(1)-(2). Although exhibits filed with his complaint include a printout of his jail trust account #3 , such a filing does not satisfy the requirement that IFP applications must be submitted on the Court's form and must include not only a printout of his jail trust account, but also a certificate section filled out by an authorized officer at his facility. See Local Rule 3.3 (N.D. Ill.). To proceed with this case, Plaintiff must either prepay the $405 filing fee or submit a completed IFP application seeking to pay it in monthly installments. His failure to comply with this order by April 10, 2024, will result in summary dismissal of this case without prejudice. See Local Rule 3.3(f).
