The plaintiff has failed to answer question 1 regarding the dates of her last employment and last monthly take-home pay. As she has applied for Disability Insurance Benefits [Dkt. #1, Par. 1], she must have been working and paying into the system at some point. Under question 3, she lists "0" for every source of income, including gifts, public assistance, and welfare. If she indeed has absolutely no source of income, she should indicate how she has exists in response to question 8.
