Filing 8 MINUTE entry before the Honorable Joan B. Gottschall: Plaintiff's amended application #7 for leave to proceed without prepaying the filing fee is granted. See 28 U.S.C. 1915(a). As required by Fed. R. Civ. P. 4(c)(3), the court orders that service of the summons and complaint be made by the United States Marshal. Plaintiff is advised that she is responsible for providing the U.S. Marshals Service with the information needed to serve the summons and complaint. Plaintiff should complete and deliver a Form USM 285, "Service of Process by United States Marshals Service."A blank Form USM 285 is available from the Clerk's Office on the 20th Floor of the federal courthouse located at 219 S. Dearborn Ave., Chicago, Illinois 60604, and from the court's website at https://www.ilnd.uscourts.gov/OnlineForms.aspx. Plaintiff has two options for submitting her Form USM 285: (1) deliver it to the Marshals Service on the 24th floor of the federal courthouse in Chicago; or (2) submit a PDF of her completed Form USM 285 using this court's online portal for pro se PDF submissions.
