ORDER: The Court denies Plaintiff's application for leave to proceed in forma pauperis #4 because he has struck out under 28 U.S.C. 1915(g). Plaintiff's complaint is dismissed with prejudice for his failure to prepay the required filing fee, failing to disclose his litigation history, and failing to advise the Court that he has "struck out." Plaintiff remains obligated to pay the $405.00 filing fee for this case, as well as all his outstanding filing fees. See 28 U.S.C. 1915(b)(1). The Court therefore orders the trust fund officer at Plaintiff's place of incarceration to make deductions from Plaintiff's account as directed below. All pending motions are terminated as moot. The Court refers this matter to the Executive Committee with a recommendation that the Executive Committee restrict Plaintiff's ability to file new civil actions in this Court until he has paid his outstanding fees in full. The Clerk of Court is directed to enter a final judgment of dismissal with prejudice. The Clerk of Court also is directed to send a copy of this order to (1) Plaintiff, at his address of record, along with a copy of the judgment; (2) the trust fund officer at Plaintiff's present place of incarceration; (3) this Court's fiscal department; and (4) this Court's Executive Committee.
