ORDER: Plaintiff Phillip Rogers' application to proceed in forma pauperis #5 is granted. The Court directs the trust fund officer at Rogers' facility to deduct $27.16 from his account and send it to the Clerk of Court as an initial partial payment of the filing fee and to continue making monthly deductions in accordance with this order. The Clerk is directed to send a copy of this order (electronically if possible) to the trust fund officer at Rogers' place of incarceration and to this Court's fiscal department. Rogers may proceed with his complaint #10 against defendant Kasperek, but defendants Moore, Fredrick, Hannigan, and Martinez are dismissed. Instructions regarding service of process will follow in a separate order. Rogers must notify the Court if his address changes, and his failure to do so may also result in dismissal of this case. ORDER: Plaintiff initiated this lawsuit without prepaying the filing fee or filing an application for leave to proceed in forma pauperis. Plaintiff must either: (1) submit a completed application for leave to proceed in forma pauperis that includes a certified copy of the trust fund account statement (or institutional equivalent) for the six-month period immediately preceding the filing of the complaint and a completed certification from the appropriate official of each institution where Plaintiff is or was confined during the last six months; or (2) pay the full statutory filing fee of $405. 28 U.S.C. 1915(a)(2). Payment should be sent to the Clerk of Court, United States District Court, 219 South Dearborn Street, Chicago, Illinois 60604, attn: Cashier's Desk, 20th Floor, and must clearly identify Plaintiff's name and the case number assigned to this case. If Plaintiff wishes to proceed IFP, he must completely fill out the enclosed form and attach a certified trust fund account statement showing activity in his account for the last six months, and have the appropriate official certify his application. Failure to comply with this order by December 1, 2025, will result in summary dismissal of this lawsuit.
