Harjani: On March 11, 2026, Plaintiff was cautioned that this case would be terminated if an amended in forma pauperis application was not filed by April 1, 2026. March 11, 2026 Filing 5 MINUTE entry before the Honorable Sunil R. Harjani: Plaintiff's application for leave to proceed in forma pauperis #3 is denied without prejudice. Upon review of the application #3 , Plaintiff did not complete sections 1 and 3. Plaintiff did not provide his date of last employment or last monthly take-home pay in section 1. Additionally, in section 3, Plaintiff referred the Court to an attached letter about his disability or worker's compensation. The one page of the letter that was provided to the Court shows two sources of income, but the letter indicates that Plaintiff may have other income that is identified on pages of the letter that were omitted. Thus, it is unclear from the attachment what Plaintiff's total income is. Plaintiff shall refile the application by April 1, 2026, with the necessary information, including information on his past employment and all income he receives from disability or worker's compensation. Plaintiff may also pay the filing fee. Not undertaking one of these two options may result in dismissal of the case. Mailed notice (lxs, )
