ORDER: Plaintiff's application to proceed in forma pauperis #3 is denied without prejudice because it is incomplete. If Plaintiff wants to proceed with this lawsuit, he must either (1) submit a renewed application on the Court's form that is certified by an appropriate official and accompanied by a copy of Plaintiff's trust fund statement showing all transactions in his account for the six-month period immediately preceding his initiation of this action; or (2) pay the full statutory filing fee of $405.00. If Plaintiff was not incarcerated, or was being held at a different facility for any of the time within the six months prior to filing suit, he must submit trust fund statements from those facilities or account for his financial status while not in custody. Failure to comply with this order by May1, 2026, 2026, will result in summary dismissal of this case.
