October 3, 2018 Opinion or Order Filing 5 ORDER:Plaintiffs application for leave to proceed in forma pauperis #4 is denied without prejudice. Plaintiff is directed to either (1) submit a renewed application that is certified by a trust fund officer and that includes a copy of Plaintiffs jail trust fund statement showing activity in his account for the six months immediately preceding his filing of his complaint; or (2) pay the full statutory filing fee of $400.00. Failure to comply with this order by November 5, 2018, will result in summary dismissal of this case. In light of Plaintiffs representation that a jail official refused to complete the trust fund certification form, the Court requests that the trust fund officer at the Whiteside County Jail grant Plaintiffs reasonable requests for information necessary to complete his in forma pauperis application. It is up to the jail to decide what policies or limits (number of copies, costs, and so on) apply when providing the certified statement. The Clerk is directed to: (1) send Plaintiff a blank application to proceed in forma pauperis and a copy of this order; and (2) mail a copy of this order to the trust fund officer at the Whiteside County Jail.
