Date Filed Document Text
March 4, 2019 Filing 7 MINUTE entry before the Honorable Iain D. Johnston: The Court denied the plaintiff's original application to proceed in forma pauperis because of information missing from his application, see Dkt. 5, and gave the plaintiff to 2/20/2019 to file a renewed application providing the missing information. To date, he has not filed a revised application. The plaintiff shall pay the filing fee by 4/1/2019. Failure to pay the filing fee by that date will result in a Report and Recommendation that the case be dismissed for failing to pay the filing fee. The clerk is directed to mail a copy of this minute entry to the plaintiff at his address on the docket. Mailed notice. (yxp, )
February 13, 2019 Filing 6 DESIGNATION of Monica V Mallory as U.S. Attorney for Defendant Social Security Administration (Mallory, Monica)
February 5, 2019 Filing 5 MINUTE entry before the Honorable Iain D. Johnston: The plaintiff's application to proceed in forma pauperis does not identify his previous employer or what he earned, and given that he has no reported income or assets, does not explain how he obtains housing, food, or any of life's other necessities. The motion to proceed in forma pauperis #3 is therefore denied without prejudice. By 2/20/2019 the plaintiff shall either pay the filing fee, or file a renewed motion to proceed in forma pauperis that identifies the information the Court has noted is missing, as well as all sources of assistance the plaintiff receives including with housing, food, clothing, transportation, medical care, and any of life's other necessities. By 2/20/2019 the plaintiff shall also filed a written report identifying the date of the Notice of Appeals Council Action letter received from the Social Security Administration. The motion for counsel #4 is denied without prejudice not only because the plaintiff has not yet established his indigence, but also because the motion does not identify any attorneys the plaintiff approached seeking representation or identify any reason why the plaintiff cannot represent himself. Any renewed motion must address the factors that favor assignment of counsel as set out in Pruitt v. Mote, 503 F.3d 647, 655 (7th Cir. 2007). Mailed notice (jp, )
