Filing 4 ORDER signed by the Honorable Philip G. Reinhard on 3/21/2019: Plaintiff's application for leave to proceed in forma pauperis #3 is denied. The application includes neither a certificate from an authorized officer (as required by this court's form application), see N.D. Ill. Local Rule 3.3(a)-(b), nor plaintiff's signature. See Federal Rule of Civil Procedure 11(a) ("Every pleading, written motion, and other paper must be signed by at least one attorney of record or by a party personally if the party is unrepresented."). The court further advises that, by signing and/or filing a document in court, plaintiff certifies that its "factual contentions have evidentiary support" and are true. Rule 11(b)(3). The Clerk of Court shall forward an IFP application form to plaintiff, who must either submit a completed, signed application (again including a trust fund statement) or prepay the $400 filing fee within 28 days of the date of this order. His failure to comply will result in dismissal of this suit. See Local Rule 3.3(f). Plaintiff is additionally advised that proceeding IFP does not mean that the court's filing fee is waived; it simply means that payment of the filing fee will be paid with deductions from plaintiff's trust fund account each month he has more than $10 in his account.
